Printed Copies
To obtain a printed copy of your medical records, please follow these steps:
- Download and print our Release of Information form – Aliso Ridge Behavioral Hospital or Release of Information form – Anaheim Community Hospital.
- Complete, sign and date the form. In order to verify your identification and validate your authorization, you are required to include a legible copy of a valid photo identification (i.e.: a driver’s license, a military ID or a state ID).
- Email, fax, mail or deliver your paperwork in person.
Medical Record Fees: There is no charge to send your medical records to another healthcare provider. For records released directly to a patient or an authorized representative there is no charge for the first 50 pages. Additional pages are charged at $0.10 per page.

Copy service/subpoena requests must be hand-delivered to personnel in our Health Information Management Department (with the exception of criminal subpoenas; via certified mail or hand-service accepted). Fees for services will be billed by a clerical hourly rate and $0.25 per page.

Urgent Requests/Records for Your Physician
For immediate continuity of care requests, you or your healthcare providers may request that records to be sent directly to their office.
The physician’s office must send a written request on a business letterhead to 949-221-3603 (Aliso Ridge Behavioral Hospital) or 714-699-5591 (Anaheim Community Hospital). Please indicate the patient’s name, date of birth and date of visit/admission. Please indicate “STAT” at the top of the request.
